How to fill in the application form (AGOS-40)

Below you will find a sample of an application form that is already filled in. The names, addresses or any numbers that you will see in these forms are not real or does not exist at all. If it is, it might be coincidence and was not meant.

Again, this is only a guide.

    i find this site very helpful. i have a question, on page 2 of the form, question 5. my principal place of practice is not the same as my residential address, so i mark NO. but what do i write on the box below? there is also a note, “if you do not yet know this address, please indicate the Australian state or territory of your principal place of practice”

    and on page 7, question 26. i did not complete my 5 year education on the recognized countries mentioned. so do i need to fill up the boxes below? or just go to the next question?

    i just want to get this right in one try. i hope you could help. thank u very much :)

  3. I do agree with all of the ideas you have introduced in your post.

    They are really convincing and can definitely work.
    Still, the posts are too short for novices. Could you
    please prolong them a bit from next time?
